Summary:
Assists with developing, organizing, coordinating and directing Facility and community resources to provide comprehensive therapeutic activities, services and programs to meet recreational, spiritual and psychosocial needs of nursing home Residents, compliant with Company Policies, State and Federal Laws and Regulations; must communicate courteously and respectfully with Residents, Employees and others and exhibit a sincere interest in working with the elderly
Qualifications:
High school diploma or equivalent; prefer two years or more of experience in healthcare Activity position; must be able to obtain and hold any required Certification; must have basic computer skills in Microsoft Office; have good oral and written communications skills; must pass a drug screen and have a satisfactory Criminal History Background Check Duties and
Responsibilities:
Commitment to providing compassionate care to our elders Assists the Activity Director as requested with interviews and Assessments of Residents prior to the initial Care Plan Conference and documentation in the Resident's Medical Record Assists with developing individual Activity Plans, Monthly Activity Program Calendars and special events Communicates Facility's Activity Programs to Residents, staff and family Assists with the organization and continuity of the Residents' Council Assists as requested with recruitment and orientation of Activity personnel Assists Residents to and from Activities; provides individual activities for Residents who can't participate in group activities Must maintain valid driver's license to assist with transportation for Residents' field trips and other situations Documents Activities and Residents' progress during MDS look-back period Assists the Activity Director with developing and complying with a Plan of Correction for the Activity Department regarding State and Federal surveys As instructed by the Activity Director, attends Family and Resident Council meetings and meets with families Must obtain and maintain any required certifications Completes Activity Sections on MDS, RAPS, Triggers and Care Plan as assigned Participates in Interdisciplinary Care Plan Meetings and Continuous Quality Improvement (CQI) Program Complies with Company Policy regarding Volunteer Workers Education:
